MSN-Psychiatric Nurse Practitioner
Lamprey Health Care - One (20 hour position) Raymond, NH and Newmarket, NH - One (20 hour position) Nashua, NH
Lamprey Health Care a leading non-profit Community Health Center in southern New Hampshire is seeking a Full Time or Part Time Psychiatric Nurse Practitioner who has a thorough working knowledge of behavioral medicine working in a fast paced health center. The ideal candidate will have experience working with an interdisciplinary treatment team including primary care medical providers. Extensive knowledge of and expertise in delivering direct psychopharmacological services to child, adolescents and adults highly preferred. Consultation to individuals who are at risk for and/or require mental health services and close collaboration with medical providers a must. Knowledgeable and extensive experience with prescribing psychiatric medications. Ability to participate and educate interdisciplinary team and medical providers about psychiatric medications and their interactions. Strong and expedient decision making abilities and accurate assessments of behavioral health conditions are required. Fast paced, patient-centered primary care environment in rural and urban sites.
Duties and
Responsibilities:
Experienced Direct Service Provider conducts behavioral health assessments using evidenced-based screening tools
Experience with using motivational interviewing with patients to adopt healthy lifestyles
Enjoys and has experience collaborating with the Psychiatrists, Primary Care Physicians, and other behavioral health staff to provide comprehensive, team-based care
Experienced in collecting patient specific health measures
Experienced with and recommends appropriate treatment plans for patients
Experienced working with external providers to arrange referrals for patients who present with diagnostic or therapeutic challenges
Experienced working with electronic medical records a plus, but will train
Supervisory Relationships: Supervised by the Medical Director (clinically) and Behavioral Health & Integrative Services Manager
Travel Required: Travel between multiple service locations may be required (Newmarket, Raymond, Nashua) if Full Time - Part Time options possible - Nashua only (20 hours) or Newmarket & Raymond (20 hours) total.
