Licensed Vocational Nurse Eureka CBOC (Per-Diem)

Eureka Community Based Outpatient Clinic, 930 W.
Harris Street, Eureka CA 95503-3927 Become an exciting member of this health care team providing compassionate healthcare to veterans who follows the Patient Aligned Care Team (PACT) model of care delivery.
We are looking for energetic, flexible and motivated LVNs.
Duties include but are not limited to:
patient education, completion of clinical reminders, injections, assisting with wound care, rooming patients, completion of vital signs and assignments related to PACT.
This involves working in active collaboration and communication with an assigned Core Team (patient, primary care provider, RN, clerk) and Expanded Team (family/caregiver, internal and community based services) in order to effectively meet the patients' needs.
The LVN will need to be able to prioritize and complete nursing care to facilitate timely patient flow, have excellent interpersonal communication skills and be a team player.
INTERMITTENT WORK SCHEDULE:
This position is INTERMITTENT (Shifts are 8 hours on days, evenings, or nights including weekends.
Intermittent staff are required to be available a minimum of four shifts per month including one weekend.
They are also required to be available for holidays).

Estimated Salary: $20 to $28 per hour based on qualifications.
