CARE MANAGER HOME HEALTH VISITS (RN) no hands on/flexible hours

Professional Edge Nursing, Inc specializes in a unique and growing aspect of healthcare. Our Care Managers use every aspect of the nursing training to empower our Members how to advocate for their own Healthcare. Our Care Managers work with our Virtual Supervisory Team to provide our clients and members with the best quality of care.
Care Managers will assess, educate, advocate, and refer on behalf of the members weekly.
Expectations: Care Managers will perform home assessments of our Members and educate them on how to advocate for their own health care needs, by doing weekly visits. Care Managers will create a Care Plan with the Member and follow it. Care Managers will be held to time sensitive situations and must document everything within 24 hours of any contact with the Members. Care Managers will have to communicate with the Virtual Team and Client Team regarding any concerns or expectations of the Members.
Requirements: Candidates will be an RN and have 1 year of Home Health Experience.
Candidates must be well organized, efficient, computer savvy, and able to work independently.
Candidates are responsible for having - Computer - Printer - Scanner (or fax)
Candidates must have Direct Deposit Accounts
Candidates must have Excellent Communication Skills - via Phone - via E-Mail - via Text - via Verbal - via Documentation
Candidates will need Professional Liability Insurance
Candidates will need to have a TB test within the last year.
Candidates will be drug screened
Candidates will have background checks monthly and prior to hire.
