Job Description:
APPLICANT REQUIREMENTS:
o LPN'S WITH MINIMUM OF 1 YEAR EXPERIENCE
oHigh School Diploma or GED Certification
oGraduation from Accredited LPN Program (Proof Required)
oActive and Clean Nursing License
oBasic MS Office (Word, Excel, Outlook)
oCare Manager MUST HAVE a computer with internet access as well as access to a printer to produce required documentation and forms for cases.
oCare Manager MUST HAVE reliable transportation.
oCare Manager MUST HAVE knowledge of Managed Care as well as Medicaid/ Medicare and how they work.
oTelephonic/ In Home customer service skills are required.
oExperience in Care Management, Skilled Nursing Facility, Geriatrics, Home Health Care, &/or Hospice.

Qualifications:
oAll applicants must provide a copy of professional license and pass a background check.
oAll applicants must sign an employment contract with our company to qualify for this position.
oOnce preliminary screening process is completed, documentation training will be provided online to all Care Management professionals that have been hired.
JOB
Responsibilities:
oTo coordinate services for members in person and assist with community resources as well as treatment needs.
oFunctioning as an advocate for members and decreasing unnecessary hospitalizations.
oTravel in assigned territory as needed to complete patient assessments thoroughly.
oIdentifying early risk factors and conducting ongoing assessments and documenting in an electronic file.
oCollaborating with the member's PCP to deliver and coordinate necessary services.
oBuilding relationships with members and their families; assisting them with proper health choices.
oEnsuring cases are documented in a timely manner.
