Judge Healthcare is actively seeking a Director of Care Navigation for a large health system within the Sylvania, OH area!This job will have the following
Responsibilities:
The director of care navigation directs the system wide strategic approach to the core functions of discharge planning and social services in hospitals and coordination of transition and ambulatory care navigation in hospitals Physicians ambulatory practice sites.
The Director ensures implementation of a best practice care coordination model and care navigation, discharge planning and social service vision and engagement.
Role is responsible to identify, assess and evaluate trends impacting care coordination, readmission, emergency room utilization, length of stay, customer satisfaction, and network capture.
They will develop and provide proactive, care coordination education to all acute, post-acute and ambulatory stakeholders.
Qualifications & Requirements:
Valid OH RN license REQUIRED Masters degree or relevant experience.
Will accept nurses currently working towards MSN Minimum of five years clinical and management experience Knowledge of care navigation, patient-centered medical home, accountable care and current reform trends preferred History of positive rapport with patients, families, physicians, and interdisciplinary team All qualified and interested candidates are encouraged to apply!!Please send your most up to date resume to Dave Casavecchia at email removed or call Dave at (484)270-1819 for more information!Thank you!.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
