Registered Nurse-HBPC (Morehead--Telework)
This position is authorized Telework.
Home Based Primary Care (HBPC) care managers are directly responsible to designated supervisor for provision of services in the home/community setting.
They are responsible for the provision of continuity of care, access, clinical guideline implementation and monitoring and performance improvement in the area of assignment.
They assess need for care and refer/collaborate with other health care providers to obtain services which provide the best overall plan for care/services.
They assure standards for home care are met and follow safety standards at all times.
- Resource for other members of HBPC when out making visits:
supply telephone, address of vet as appropriate, get in touch with provider for urgent needs of staff when in a veteran's home, intercede in information passage during a crisis with staff on visits- Monitor post hospital calls- Contact the Veteran to provide lab results, get narcotic counts, make appointments for provider to make home visit- Troubleshoot all calls, solve or route appropriately, follow-up those calls to see if the problem has been resolved WORK SCHEDULE - Monday through Friday 8am - 4:

Estimated Salary: $20 to $28 per hour based on qualifications.
