Care Management Coordinator-Nurse or Social Worker Only

Responsible for providing care management for a group of members through a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet a member's health needs through communication and available resources.
Managed Long-Term Care Plans (MLTCP):
Provide long-term care services (like home health and nursing home care) and ancillary and ambulatory services (including dentistry, and medical equipment), and receive Medicaid payment. Members get services from their primary care physicians and inpatient hospital services using their Medicaid and/or Medicare cards. Members must be eligible for nursing home admission. While several plans in New York State enroll younger members, most managed long-term care plan enrollees must be at least age 65.
Job Requirements
1. Registered nurse with current New York State licensure to practice professional nursing. BSN preferred. Or
current registration/license to practice as a Social Worker in New York State with a Master's Degree in Social
Work from an approved program. National Committee for Quality Assurance NCQA certification preferred.
2. One to two years certified home care agency or long term home health program experience or one year
experience in a similar role working in a manage care plan environment.
3. Complies with acceptable professional standards and practice.
4. Ability to market and deal tactfully with members and the community.
5. Knowledge of corporate business management and managed care environment.
6. Demonstrates a working knowledge of best practice clinical skills, good communication and public relations
skills.
7. Demonstrates computer proficiency. In addition, the ability to learn and adapt to multiple MLTCP software
programs.
8. Excellent organizational and communication skills.
9. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job
responsibilities.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
