The teams of professionals work closely with the physician to coordinate care in the patient s home or place of residence.
The focus of care is to provide the patient with the home care and support they need to achieve quality of life, maximizing your independence to the highest possible level.

The department Secretary is responsible for:
Providing administrative support with respect to admissions, discharges, transfers, and recertification of patients Providing high quality customer service Providing administrative support for day to day operations of the team Scheduling Home Health Aides and RNs' home visit appointments Requirements:
A high school diploma or the equivalent is required.
All new employees must s successfully complete department specific orientation.
Current clerical experience in a Health Care facility is preferred Must have excellent computer skills and a professional manner Good communication and interpersonal relationship skills required.
May occassionally be required to work on a weekend.
Estimated Salary: $20 to $28 per hour based on qualifications.
