Licensed Practical Nurse (LPN) Case Manager
We are not a Staffing Agency; our employees are Full-time and Part-time Employees.
We currently have full-time Licensed Practical Nurse (LPN) positions available for Transitional Care.
Essential Job Functions:
Ensure continuity of care for patient transitioning from hospital to home care or hospice
Initiates plan of care for patient
Identifies appropriate patients for TCM prior to hospital discharge utilizing established criteria
Round in the hospital with hospitalist group to determine appropriate patients
Enroll patients into the TCM program and obtain all necessary documentation
Serve as a guide to the patient, coaching them with self-management tasks.
Evaluates aspects of each patient's condition, diagnosis, medications and support systems to formulate an individualized plan which will lead to successful outcomes in medication self-management, use of dynamic patient centered record, appropriate primary care and specialist follow-up, knowledge of red flags.
Serves as the bridge between the professional staff in a care setting, (e.g. hospital, and the patient and/or family and the Transitional Care Liaison.
Collects and analyzes data on clinical indicators to identify opportunities for improvement.
Participates in the orientation of new personnel, mentors peers and promotes collaborative teamwork.
Facilitates Telemedicine visits as assigned
Facilitates Chronic Care Management
Job Requirements:
Must possess excellent communication, critical thinking, negotiation, and conflict management skills.
Experience with electronic health records (EHR).
CPR certified.
Ability to work independently, while collaborating with other team members.
Ability to make independent decisions in accordance with established policies and procedures.
Computer proficiency including, but not limited to, data entry, retrieval, and report generation.
Requires the ability to read, write and speak effectively in English.
Knowledge of patient appointment scheduling.

Qualifications:.
Completion of training.
Current and in good standing Licensure for practicing as a Licensed Practical Nurse (LPN) in the State of Arizona.
Prefer 1-2 years of experience working as a Licensed Practical Nurse (LPN).
Clinical training in a medical or clinical environment
Chronic Disease management training
Knowledge of evidence-based clinical practice guidelines
Case management/Discharge planning
With our unique model, our current full-time Licensed Practical Nurses are able to make 54K to 78K for seeing 10 patients per day in the hospital.
Full Time position, 5 days a week. Contracted with majority of health plans. For full-time employees we have full comprehensive benefits including health insurance, life insurance, disability insurance, 401K, malpractice with tail coverage, in house credentialing/billing, up to 4 weeks off per year, mileage coverage, and flexibility in scheduling.
MD24 House Call is a largest network of Primary Care Clinicians and Physician Specialists who provide mobile medical services, either in-person or via telemedicine, to patients in assisted and independent living communities including numerous Memory Care facilities. Our services are specifically designed for the elderly populations. You will start with immediate full patient loads.Estimated Salary: $20 to $28 per hour based on qualifications.
