LPN Case Management
Healing Touch Home Healthcare is looking for LPNs to case manage clients in Lima and surrounding areas. This opportunity provides a flexible schedule, paid mileage, ability to work closely with patients and physicians.
Must be able to travel around the Lima area and have reliable transportation.
Applicants must be able to pass a Background Check
Excellent communication and documentation skills.
Must be knowledgeable of nursing and medical practices, as well as laws, regulations, and guidelines that pertain to home health care.
Candidate should have experience with computers and access to the internet.
Candidate must have LPN license and would have 1
years of experience in home health care, hospital setting or long term care facility. Potential hire must possess leadership, teaching and supervisory ability.
The pay-rates for the Humana program are:
The initial visit for each new client is $20.00 per visit, and $15.00 for 1 hour of documentation at home (The initial visit normally lasts 1 - 1.5 hours)
The follow-up weekly visits for each client is $20.00 per visit (The follow-up visits normally last between 15-20 minutes) (Since the visit rate covers an hour of time, documentation can be completed using the remaining time.)
We also pay mileage .25 per mile (to and from clients)
