Registered Nurse (Nurse Navigator)

The Nurse Navigator will be aligned within the Integrated Fee Service and will coordinate non-VA care to assure that care is timely, appropriate, of high quality and cost effective.
A Nurse Navigator works closely with the primary care provider (or providers), other healthcare professionals and team members, other clinics, internal or external services and community agencies.
Role Responsibilities and Accountabilities:
The Nurse Navigator is directly responsible to the Nurse Manager of the Non-VA Care.
The incumbent utilizes nursing skills and knowledge in an independent manner in areas of patient assessment and planning.
The incumbent collaborates with the interdisciplinary health care team, community agencies/facilities, and patient/families/significant others to ensure continuity of care across all levels of care insuring timely placement of VA patient into the appropriate level of care.
The goals of the non-VA care are to improve continuity of care, including appropriate, high-quality disease management, as well as improve patient satisfaction, increase access to services, and reduce resource usage thus decreasing health care costs.
Roles and responsibilities and accountabilities also include demonstration of knowledge and skills necessary to provide care appropriate to the population related needs of patients served in the assigned area of practice.
This knowledge includes understanding changes associated with aging and special populations, i.
e.
women Veterans, OEF/OIF, Vietnam, Desert Shield/Storm, Korea, and World War II, etc.
Duties include but are not limited to:
1.
Participates in Peer Review Process 2.
Initiates electronic consults for all faxed requests for extensions or additional services.
3.
Manages non-VA care to include coordination of scheduling for services that can be provided within the VA health care system.
4.
Coordinates VA and non-VA care to ensure high quality health care is provided.
5.
Ensures that medical documentation is received in a timely manner 6.
Scans records and closes electronic consults.
7.
Coordinates with non-VA for post-discharge needs, including prosthetics (DME, home oxygen), home health, etc.
to ensure that Veterans discharged from non-VA care receive appropriate medical supplies.
8.
Provides professional assessment, coordination, and planning of multiple health care services; acts on the behalf of the Veteran to assure that necessary clinical services are received, and monitor progress.
9.
Provides ongoing evaluation of care management services.
10.
Communicate and interact appropriately and courteously with all internal and external customers.
11.
Maintain confidentiality of electronic, written, and/or verbal patient/employee information.
12.
Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment.
13.
Other, related duties.

00 am to 4:

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
