RN Intake Coordinator-Home Health

The Rideout Home Health intake coordinator is responsible for the referrals that are phoned, faxed or otherwise referred to the Home Health office.
This includes a variety of duties including , but not limited to, knowledge of state and federal regulations, obtaining physician orders and Face to Face documents, organizing paperwork and data input, coordinating services for staff and on-site visits to physicians, care coordinators, and hospital as needed.
KNOWLEDGE:
Graduate of an accredited school of professional registered nursing.
SKILLS:
Ability to use sound judgment, work autonomously, but recognize own limitations.
Good technical/computer skills.
Diplomatic in dealing with patients, physicians and other staff and to deal effectively with stress.
Ability to plan and organize workday allowing for flexibility in assignments and due dates.
Strong clinical background to process the referral information and coordinate the care of the patient.
Experience:
Minimum of one year experience in home health care or equivalent health care setting.
LICENSE:
Valid California Drivers License.
Current auto insurance.
Current California license for RN.
Estimated Salary: $20 to $28 per hour based on qualifications.
