Clinical Pharmacy Specialist (Acute Care Medicine)

The Clinical Pharmacy Specialist functions at the highest level of clinical practice, working independently with prescribing privileges under a scope of practice in the acute care setting.
The Clinical Pharmacy Specialist is fully responsible for planning, developing and maintaining highly innovative and progressive clinical pharmacy programs in their assigned area by implementing rotational cost effective drug therapy to improve patient outcomes.
Duties include but are not limited to:
o Designing, implementing, assessing, monitoring and documenting therapeutic plans o Helping achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams o Ordering laboratory tests to help determine efficiency and toxicity of medication therapy o Utilizes the highest level of clinical privileges for clinical pharmacy specialists o Participates in inpatient rounds with medical/ surgical services o Provides comprehensive therapeutic drug monitoring for inpatients on Vancomycin and Aminoglycosides o Manages inpatient Warfarin dosing and monitoring by initiating dose changes, ordering labs and documenting activity in CPRS o Provides consultation on selection of appropriate dosage regimens as follow-up when non-therapeutic drug levels are obtained for inpatient medical/ surgical patients o Performs medication reconciliation for patients at the time of discharge o Serves as an expert in clinical pharmacy practice to the acute care staff, responding to requests from practicing clinicians regarding difficult therapeutic problems with a high degree of complexity o Exhibits a high degree of accuracy in all areas of drug accountability o Knowledge of simple procurement requirements to assure that a drug can be acquired during the evening, weekend and holiday hours o Participates in the development and enforcement of Pharmacy and Health system policies and procedures o Attends monthly and/ or as needed clinical pharmacy meetings o Maintains all pertinent professional and administrative records as necessary o Serves as a preceptor to pharmacy residents and pharmacy students during rotation o Participates fully in providing timely verbal and written feedback, evaluations, and documentation of pharmacy residents utilizing Resitrak software o Submits review articles or case studies for publication in appropriate professional journals and develops presentations for local, state, and national meetings o The pharmacist will maintain strict security over all medications in the Pharmacy as well as in any medication storage areas within VACT o Protects data from unauthorized release of information, alteration or deletion o Listens to all customer/ patient feedback, positive and negative, acts to resolve complaints within his or her control, and reports feedback to management in a timely manner Work Schedule:

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
