Description:
Provides physical therapy services under the direction of the attending physician.
Works within the guidelines set forth by the Clinical Coordinator, in scheduling visits, admissions to and discharges from agency services.
RESPONSIBILITIES 1.
Performs initial assessments and assesses for appropriateness of admission to home care.
2.
Formulates realistic short and long range treatment goals based upon assessment of patient needs and evaluates progress in achieving the desired goals.
3.
Plans and administers individualized patient physical therapy programs as prescribed by the physician and in accordance with agency policy.
4.
Carries out a physical therapy treatment program under specific and current medical orders.
5.
Applies Physical Therapy procedures such as:
o Therapeutic exercises, muscle reeducation o Physical agents, including heat, cold, water, light, electricity and massage o Functional and ADL training o Transfer and gait training o Prosthetic and orthotic training 6.
Treats patients to relieve pain, develop or restore function, and maximize potential for independence in physical activity.
7.
Observes, records and reports to the physician and agency personnel the patient s reaction to treatment and any changes in patient s condition.
8.
Recommends and demonstrates home adaptation equipment and home exercise programs involving family members and agency staff who may assist with the program.
9.
Instructs patient and family in the use of home equipment, orthotics and prosthetic devices.
10.
Maintains appropriate records on each patient.
Prepares initial evaluation, clinical notes, progress reports and discharge summary.
11.
Establishes, when appropriate, and instructs patient and/or family members in a program of maintenance therapy/exercises for continuation beyond the therapy treatment program.
12.
Demonstrates knowledge of home care services and programs with appropriate referrals for additional services through RN Supervisor or the Clinical Coordinator.
13.
Coordinates with RN Supervisor or Clinical Coordinator for continuation of care or discharge with notification and approval of referring physician.
14.
Participates in case conferences and Quality Assurance as requested; attends agency meetings and continuing education programs as appropriate.
15.
Maintains patient confidentiality.
16.
Complies with all Federal, State, local and JCAHO standards, rules and regulations.
? Requirements:
A graduate of an approved school of physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or the equivalent.
A current New Jersey license to practice Two years experience in rehabilitation / home health care is required.
Excellent knowledge of physical therapy modalities and treatment procedures is necessary.
The ability to be self-directing in relatively unstructured situations.
Must possess effective verbal and written communication skills.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
