Clinical Pharmacy Specialist (ICU)

This position is located in the Pharmacy Service of the VA Connecticut Health Care System (VACT) at the West Haven campus.
The clinical pharmacy specialist is fully responsible for planning, developing, and maintaining highly innovate and progressive clinical pharmacy programs in the intensive care unit patients by implementing rational cost effective drug therapy to improve patient outcomes.
The incumbent assists with clinical education activities, provides drug information, contributes to medication use evaluation (MUE) activities, and provides support to the Medication Management Committee(MMC).
The major duties include, but are not limited to:
Participates in inpatient rounds in the medical intensive care unit and with the medicine service.
Writes orders for medication and laboratory orders in the Computerized Patient Record System (CPRS) under a scope of practice.
The incumbent will independently manage patients on medications requiring intensive therapeutic drug monitoring to maximize patient response and minimize drug interactions and adverse drug effects.
Manages inpatient warfarin dosing and monitoring by initiating dose changes, ordering labs, and documenting activity in CPRS.
Performs medication reconciliation for patients at the time of discharge.
Performs all pharmacy distributive functions according to the VHA and local policies to provide comprehensive pharmaceutical services in a timely fashion to meet the needs of our patients and medical center staff.
Demonstrates knowledge of all established pharmacy department policies and procedures, guidelines and protocols and applies this knowledge accordingly.
Demonstrates knowledge and ability to perform routine maintenance on all equipment(manual and electronic) throughout the Pharmacy department.
Ability to prepare sterile IV products, including but not limited to chemotherapeutic agents.
Ability to perform all vault duties when processing controlled substances for inpatient or outpatient dispensing.
Participates in the development and enforcement of Pharmacy and Health System policies and procedures.
Assists in the dissemination and enforcement of guidelines established by the Medication Management Committee regarding the use of formulary, non-formulary, and restricted drugs.
Assists in designing, conducts, and participates in studies, audits, and evaluations concerning the utilization review of drugs the auspices of the Medication Management and Quality Assurance Committees.
Performs other duties as assigned.
Work Schedule:
Rotating Shifts including nights, weekends, and holidays.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
