MCI Framingham - Section 35 Aftercare Coordinator
Qualifications: A minimum of a bachelor's degree in one of the following disciplines or a closely related field: psychology, education-counseling, clinical psychology, social work, rehabilitative counseling; and has a minimum of one year of supervised substance abuse counseling experience or has a recognized certification or licensure. Must be knowledgeable in substance abuse, mental health, and community agencies in order to facilitate placements.
REQUIREMENTS:
o Must present as courteous and professional at all times;
o An understanding of the disease concept and current therapies and/or self-help programs used to treat addiction;
o Demonstrates flexibility and ability to perform multiple tasks;
o Must have excellent communication skills, including the ability to organize ideas in a logical and clear fashion;
o Must have the ability to work with a team in a cooperative and collaborative manner;
o Participate in intra-agency collaboration interfacing with staff, civil commitments, and residential facilities including residential programs.
o TB screening;
o A thorough understanding of boundaries and ethics;
o Certification in the HPTC approved de-escalation program and all required MCI trainings;
o Must have the ability to be productive in difficult situations with good problems solving abilities.
Responsibilities:
o Compliance with Federal Regulations, 42CFR Part 2, Confidentiality of Alcohol Abuse Policy and HIPAA Regulations;
o Participation in staff meetings, case conferences, and educational conferences for the enhancement of professional and personal growth at both MCI and HPTC;
o Individual meetings with the civil commitments to determine reentry needs;
o Collaboration with community agencies to make referrals regarding reentry needs and to set up transportation arrangements when final discharge arrangements have been identified;
o Responsible for linking the civil commitment's with aftercare services prior to discharge and aiding the civil commitments and family to access outpatient services where appropriate:
o To arrange telephone intakes and/or face to face interviews with halfway houses / residential facilities by following the visitor process;
o Attend all unit, facility meetings, treatment team meetings and case conferences as determined by the Treatment Director/Program Director;
o Responsible for continued education in the field of substance abuse and related courses;
o Attend mandatory trainings, and monthly supervision at WATC;
o Completes appropriate documentation in the IMS system in a timely manner, i.e. progress notes, transportation request slips;
o Enters data into the EIM/ESM as assigned;
o Collaboration with WATC staff when transferring a client from MCI to WATC;
o Communicate with family after waivers signed and document all interactions in IMS;
o Ensure compliance with civil review process;Estimated Salary: $20 to $28 per hour based on qualifications.
