Position
Summary: In order to support and help create positive health outcomes for patients, the Clinical Care Coordinator provides assessment, intervention, coaching and support to MCPN patients to proactively manage their care.
Essential Duties/
Responsibilities:
Coordinates the delivery of services to patients, including collaborating with clinical and other staff to address patients' needs in clinic/home/hospital settings as appropriate.
Assesses barriers to health and determines appropriate ways to address those barriers.
Evaluates patient's condition and develops care plans to improve overall health situation.
Offers the patient guidance and education related to their specific needs.
Connects patients to resources within MCPN and the community to address identified health concerns.
Participates in case review sessions.
Provides timely documentation and required reports for all care coordination activities.
Provides input into the service delivery systems for Accountable Care within MCPN, based on evidence-based models of care coordination (e.g., Eric Coleman, Camden, etc.). Service delivery systems include coordination of referral systems, and interfacing with internal clinical and non-clinical services and programs.
Provides input into care coordination protocols and procedures based on evidence-based models
Provides mentoring and assists with skill building for entry-level care coordinators as needed
Job Qualifications - Knowledge, skill and ability:
Ability to interact positively and build rapport with patients, coworkers and/or external contacts.
Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of MCPN's diverse patient population.
Ability to handle sensitive information ethically and responsibly.
Ability to protect the confidentiality of patient, employee and business information.
Ability to discern information from others in a variety of formats and communicate information to others in a manner that helps them understand instruction.
Ability to work independently in a manner that ensures accuracy and efficiency.
Ability to demonstrate empathy with potential cultural and diversity dynamics.
Demonstrates advanced problem solving skills.
Ability to utilize advanced customer service skills, including the ability to diffuse upset patients.
Education or Formal Training:
B.S. or B.A. degree in Human Services or related field required
Direct related care coordination experience may be substituted on a year-for-year basis for the bachelor's degree.
MSW / LCSW preferred.

Experience:
Previous experience working in social work, behavioral health or community health required.
Background in case management, health education, or community resources required.
Bilingual Spanish required

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
