Assigned to the Geriatrics and Extended Care Service LIne (GEC), this Social Worker will provide a vast array of professional social work services to the Veteran population admitted to Manchester VA Medical Center's Community Living Center (CLC). This includes long and short term care on the CLC and also to Veterans and families admitted to the Palliative Care Unit (PCU). The duties include :
Case management, clinical treatment and advocacy.
Coordination with other VHA programs and community agencies.
Assessing and treating the psychosocial needs of veterans and their families admitted and transitioning from CLC.
Collaborating with other members of CLC, PCU, and PACT and other interdisciplinary teams.
Providing other GEC SW services as needed.
You will also perform the following services and duties as they pertain to case management and individual patient treatment:
_Psychosocial Assessment_
_Community and state resource and Development_
_Case Management_ and discharge planning/coordination
_Advocacy_
_Crisis Intervention_
_Education/Health promotion and prevention_
_Coordination of VHA non-institutional and institutional services_
_Supportive counseling_
Coverage: The incumbent provides and can expect coverage from other social workers. In unusual situations the social worker may be required for coverage outside of the GEC service line and if needed, in a rotation system of social work on-call to the medical center.
To qualify for this Social Work position you must:
Be a citizen of the United States; and
Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council
on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. _A doctoral degree in social work may not be substituted for the master's degree in social work_ ; and
Must be licensed or certified by a state at the advanced practice level to independently practice social work; and
Social Workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
Must have valid driver's license for local travel

Physical Requirements: Lifting and carrying up to approximately 25 lbs., manual dexterity, walking less than 1 hour, standing 1-2 hours, sitting for 2-6 hours per day. Work is primarily in an office, clinically based setting. Typing/computer based work occurs on a daily basis.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
