While VMC Department of Pharmacy has enjoyed a progressive and comprehensive pharmacy presence for many years, we too were moved by the call from ASHP and other industry leaders to prepare for the future. In 2011, a focus group was formed to address the urgent need to proactively plan for changes in healthcare reform. Following a thorough evaluation of our current practices and systems we began the process of designing a new practice model that would support PPMI Summit consensus recommendations and meet the needs of our patients. Final plans are underway for a multiphase expansion and redefinition of pharmacy services. The new, world-class practice model expands on our existing framework of robust clinical services and provides for both decentralized unit-based and service-based clinical pharmacy services via a system of seven integrated teams. These teams, comprised of both pharmacists and technicians, are supported by a dynamic central pharmacy team responsible for production of sterile products, inpatient chemotherapy, cart fills, inventory and automation. Most drug distribution is automated, using Robot-RX, medical carousels and unit-based automated dispensing cabinets.

Responsibilities
Serves as a primary preceptor for UNC Eshelman School of Pharmacy students on Ambulatory Care clerkship and provides other teaching, mentoring, and student support collaboratively with the VMC Department of Pharmacy.
Follows UNC Eshelman School of Pharmacy and departmental guidelines for reporting learner issues by communicating with local faculty liaison.
Provides didactic lectures, teaching in seminar and serves as a student mentor or organization advisor, if requested
Engages in scholarly activities and assists with patient-focused outcomes research with students and residents at VMC
Coordinates PGY1 resident-facilitated Transitional Care experience and serves as the primary preceptor this residency rotation.
Maintains clinical practice skills and competently functions in a staffing role. Staffing includes every 3rd weekend, holiday rotation and some 2nd shift.
Accountable for the safety and quality of pharmacy services delivered for area.
Actively participates in department and hospital wide continuous quality improvement programs and works toward excellence.
Reports and manages medication errors to promote patient safety
Enhances quality of delivery of care by supporting the mission, vision, and values of VMC and Department of Pharmacy.
Engages in active patient-focused practice that includes the provision of all components of transitional care with the goal of improving outcomes for identified patients at VMC.
Ensures that accurate medication histories are obtained upon admission in the patients identified
Assures that patients' written and verbal medication instructions at discharge are accurate and clear
Ensures that discharge medication orders are communicated with the patient or caregiver, the community pharmacist (when applicable), ambulatory care pharmacist or other community resource
Oversees the process of post-discharge follow-up
Works collaboratively with Case Management and Discharge Planners to provide integrated care for the patients served
Evaluates barriers to medication access
Provides appropriate documentation for all transitional-focused or medication therapy management interventions
Collects data on established metrics that evaluate impact of pharmacy-directed transitional care
Evaluates outcomes data
Develops and provides quality precepting experience for learners on Ambulatory Care/transitional care clerkship at VMC
Supports educational mission of VMC and Department of Pharmacy through training of students and residents
Communicates drug information to physicians, nurses, pharmacists, and other members of the health care team
Completes other duties to support departmental operations as assigned or directed.
Minimum Requirements
BS, MS or PharmD in Pharmacy
NC Pharmacy license or eligible for licensure and licensure within 2 months of employment

PGY1 residency training highly recommended; PGY2 residency training and/or precepting experience preferred

Estimated Salary: $20 to $28 per hour based on qualifications.
