Job Details:
The Social Worker I HomeCare provides social work services to homebound patients in order to assist in alleviating the social and emotional problems directly related to the patients' illness or current medical problem. These services include but are not limited to the identification of eligibility for state and federal programs, assistance in accessing applicable programs, and providing emotional support related to the patient's situational reaction to their current status.
Qualifications/Competencies:
ME Licensed Social Worker (LSW) required
Bachelor's degree in social work or related field and current state licensure as LSW. BSW preferred
At least one year of social work experience in a health care setting required. Home Health experience preferred
