Job Details:
Minimum 1 year experience required
Under the general direction of the Medical Director, the Palliative Care LCSW is responsible for providing clinical social work services to seriously ill patients and their families. Coordinates palliative services and interdisciplinary meetings and communications, and maintains statistics and leads program quality initiatives.
Qualifications/Competencies:
Current Maine State Board of Social Work Licensed Clinical Social Worker (LCSW) required
ACHP Certification
Master's degree in LCSW preferred
Up to 1 year or more: dedicated prior work experience with palliative care patient population preferred
Ability to work in a dynamic environment with multiple priorities
Able to work with others at all levels of the organization
Ability to competently organize, facilitate, educate, document and work effectively with patients, families, physician, manager, administrators, community contacts and other disciplines.
Ability to use electronic medical records system.
Ability to work independently, effective time management and problem solving skills, able to manage multiple priorities.
Detail-orientated, with strong organizational and ability to multitask
Must have confident and effective oral and written communication skills
Proficiency in a suite of Microsoft Office applications
