Summary:
PRIMARY PURPOSE
To promote efficient coordination of patient care through collaborative practice with the Healthcare Team. To promote patient, family, staff and physician satisfaction through coordination and continuity of care. To provide a systematic approach to evaluation of patient care. To coordinate patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness.
DUTIES AND RESPONSIBILITIES
Under the direct supervision of the Manager, Case Management and within established policies and procedures, the incumbent performs the following functions:
Essential Functions:
Patient Care Facilitation
Collaborates and participates with the primary nurse, social worker, and the multidisciplinary team in the ongoing clinical assessment of patient status and care management.
Utilization Management
Reviews patient admission information within 24 hours and determines patient s appropriateness for acute hospital level of care, Assesses patient s clinical course to verify patient s continued need for acute hospital level of care.
Quality Management
Conducts assigned quality management activities to improve patient care and service. Participates as part of the interdisciplinary team in the development or revision of hospital policies /procedures pertaining to patient care processes.
Discharge Planning
Performs initial chart review for all patients for early identification of discharge planning needs and facilitates planning in collaboration with the interdisciplinary team, patient and family, Serves as a resource to the interdisciplinary team for discharge planning options through daily rounds and discharge planning rounds.
Education
Maintains current knowledge of: Managed care requirements for utilization of services, and available community resources.
Qualifications:
Education: Graduate of an accredited school of nursing. BSN or Bachelors in health related field or equivalent work experience.
Experience: Minimum of two years experience in case management and/or utilization management required, minimum of two years of quality management experience preferred. Three years clinical experience.
Licensure/Certification:Current license as a registered nurse in the state of New Hampshire.
