Essential Functions & Responsibilities
Provides information and referral services for organization patients/families/caregivers regarding practical and environmental needs.
Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying.
Identifies and refers patient/family/caregivers to appropriate individuals or agencies as necessary.
Serves as liaison between patient/clients or family/caregivers and community agencies.
Participates in discharge planning when needed. Assists patient/family/caregiver with securing durable power of attorney and with funeral arrangements as needed.
Qualifications/Experience
Masters degree in social work with accreditation by the council on Social Work Education and current Iowa State License
One year clinical experience in health care.
Strong verbal and written communication skills.
Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
Ability to work as a collaborative team member.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
