The S.N.1st program
The Physician will travel locally to the Surrounding areas of Springfield or Auburn, MA to visit elderly patients in compliance with the S.N.1st Program.
Currently seeking a (2) Gerontologist or IM/FM GER/MED certified physicians interested and committed to providing specialized care for our geriatric population. You will work independently with the help of P/A and N/Ps mid-level providers in a robust outpatient practice with an established referral system.
Our physicians work collaboratively with the Advanced Practice Clinicians, an RN case manager and a behavioral health specialist to provide patients a personalized, patient-centered care plan in addition to General medical treatment (Internal medicine on site that works with patients)

These are out patients and you will see 15-20 a day.
The clinic is staffed by a full-time nurse.Estimated Salary: $20 to $28 per hour based on qualifications.
