The RN participates as a member of the health care team in delivering quality health care to patients, which supports Kaiser Permanente's mission, vision and values. The RN provides a variety of nursing services both in person and by phone utilizing the nursing process and leadership skills to address acute and chronic needs of Health Plan members and other patients of the Kaiser Permanente Health Care Program. He/she works under the general direction of the designated supervisor.
Essential Functions:
Support the MH Prescriber team members by responding to patient messages related to psychotropic medication problems, questions, or routine reports; concerns about mood or behavior; questions regarding lab results; or other urgent issues that fall within the nursing scope.
Assess & evaluate patients based on the DSM IV, providing telephone advice independently & within scope of the Oregon & Washington Nursing Practice Act.
Triage patients as needed.
Provide patient & family education regarding side effects & efficacy of psychotropic medications & behavioral management.
Facilitate care coordination with the patient's psychiatrist, primary care provider, other treating providers, lab, pharmacy, & others as needed.
In collaboration with the patient's treating prescriber, provide outreach services to high risk patients.
Outreach to patients as frequently as needed, ensuring patients are receiving appropriate services corresponding to the patient's risk level.
Triage & coordinate care as needed.
Assist the local MH team in hospitalizing patients, searching for available beds, arranging for transportation if needed, & ensuring patient transfers occur successfully.
Assist patients in crisis, either by telephone or in person.
Job duties specific to the Float Mental Health RN role:
The Float RN will NOT be assigned to a regular group of psychiatric prescribers.
The Float RN will cover calls on the benefit days off for the non-float RN team members or cover calls for the Team Lead during designated administrative time.
The Float RN may support the Doctor of the Day on days of unusual call activity.
Job duties specific to the Mental Health Advice RN role:
Represents the role of the Mental Health Advice Nurse.
To strengthen the linkage between the assigned psychiatric clinicians & Kaiser Permanente members by responding to patient messages related to psychotropic medication, mood or behavior, test results or other issues that fall within the nursing scope of practice.
To assess & triage patients based on the current Psychiatric Diagnostic & Statistical Manual providing telephone advice independently & within scope of the Oregon & Washington Nursing Practice Act.
To provide patient & family education regarding side effects & efficacy of psychotropic medications as well as behavioral management.
To facilitate care coordination with the patient's psychiatric clinicians, primary care provider, other treating providers, laboratory, pharmacy & others as needed.
To assist the MH team with hospitalization of patients.
To provide care coordination in assisting patients through the levels of care provided within the mental health system.
The MH RN is cross trained in Advice /Triage & Case Management roles & responsibilities.
Goals include: increasing access to services, increasing member satisfaction, providing transitional support for Kaiser Permanente members & their families & ensuring continuity of care for high risk members at various levels of care such as inpatient hospitalization, residential care, outpatient programs & community supports.

Qualifications:
Minimum two (2) years of experience in acute care or ambulatory care/clinic setting within the last five (5) years.
Two (2) years of psychiatric nursing experience.
High school diploma or GED.
Graduate of Accredited School of Nursing.
Current RN license in Oregon and Washington. Requires Oregon or Washington license at time of hire. Licenses in both states are required within 6 months of hire.
Basic Life Support (BLS) for Healthcare Providers.
Demonstrates clinical nursing and leadership skills.
Able to type 20 words per minute.
Demonstrates effective interpersonal, communication and problem-solving skills.
Willingness to learn computerized information systems.
Demonstrates ability to work within teams and within a dynamic work environment.
Broad-based knowledge of Mental Health diagnoses and treatment, including psychotropic medications and associated lab monitoring.
Knowledge of contributory and/or co-existing diagnoses such as chronic pain, fibromyalgia, eating disorders, and chemical dependency.
Knowledge of risk factors of suicidality and lethality.
Basic diagnostic assessment through the use of the DSM V.
Skills and ability to assess suicidality, lethality, and to handle crisis situations.
Customer focused.

Previous experience with population care/case management, triage and advice preferred.
Current or future bachelor's degree in nursing or a related field preferred.
Certification in psychiatric and mental health nursing and/or nursing case management preferred.
