Master Social Work Supervisor Western Nebraska Veterans' Home is seeking an experienced and motivated, Master Social. Work Supervisor for full time Day position. Requires Master's degree in Social Work and provisional Certification as a master social worker from the State of Nebraska. Responsibilities include social case with personal issues involving financial, health, family, and environmental adjustment. Social history data collection to assess al, vocational, and social needs. Will serve as advocate for the Home's members by coordinating referrals to various service agencies. Participate in interdisciplinary care plan team meetings and MDS assessments. Conducts discharge planning with the client, family/guardian, and treatment team. Documents and reports client's social activities and maintains necessary records in the client files.
