Conifer Health Solutions is currently hiring an RN Case Manager who lives in the Framingham/Worcester, MA area.
JOB SUMMARY

By following the principles of mutual respect for patients and families, our Nurses work with participants, providers, employers, health systems and hospitals, payers, professional advisors and communities to identify and coordinate the most appropriate services to address their healthcare needs.
As a technology based company, Conifer has developed a comprehensive, fully integrated, URAC certified web-based medical management software application that combines our web-based data warehouse and reporting platform with a robust medical management software and workflow system. The result is a seamless and comprehensive database, management and reporting tool that will support all levels of medical management interventions; utilization management, case management, and disease management. Our nurses find this application to be a very efficient, easy to navigate and helpful tool as it is integrated with documentation and research materials.
KNOWLEDGE, SKILLS, ABILITIES
Ideal candidates are able to:
Utilize innovative tools and provide a wide range of medical management services, which include utilization, case and disease management.
Analyze the data provided through the tools in order to identify gaps in the health of participants and populations, predict participants at risk and improve outcomes.
Work cooperatively with employer groups, physicians, managers, nurses, allied health staff and external agencies.
Good presentation and communication skills
Ability to travel among facilities
EDUCATION / EXPERIENCE
US Citizenship required
Must hold a Massachusetts RN license
At least three (3) years diverse clinical experience as an RN is required and experience in Case Management
Bachelor's degree is preferred
Five (5) years UM/CM experience if no bachelor degree
CCM / CPHM / UM certification preferred
Strong aptitude for computers- basic level skills will not be sufficient
Spanish speaking is a plus
Ability to attend a 4 day orientation in Annapolis, Maryland (expenses paid)Estimated Salary: $20 to $28 per hour based on qualifications.
