Job Details:
Provide Hospice Care to terminally ill individuals and their families.
Contributes to the overall integration and success of the Program and the Network by being a team player.
Promotes organizational integration based on the mission, vision, and values. Complies with the Network's Corporate Compliance Policy as set out in the Standards of Ethical Conduct and Business Practices.
Requirements: Master's degree from a school of social work accredited by the Council on Social Work Education and current licensure as an LMSW, LMSW Conditional, LCSW or CSW-IP.
At least one year of social work experience in a health care setting required. Home health or hospice experience preferred.Must have validMainedriver's license.
