Regisered Nurse Case Manager - Case Management

Responsibilities
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients on their assigned unit
Participates in department performance improvement
Develops professional knowledge base
Participates in care coordination rounds
Demonstrate awareness of patient rights including confidentiality, privacy, informed consent, surrogate decision making and advance directives
Performs comprehensive psychosocial assessment of patients
Counsel patients and families around the issues of emotional adjustment to illness, disability, hospitalization and/or discharge plan
Demonstrate the knowledge and skills necessary to determine age specific needs
Provides information and referral services to patients, families and community callers
Assist patient in securing financial assistance when needed
Demonstrates working knowledge of community resources
Collaborates with CM partner to establish appropriate plans for patient at discharge
Interacts with patients, families, and health care team to develop proactive plans for continued care and discharge planning.
Documents assessment and interventions in the medical record
Provides support for crisis intervention, bereavement counseling, and substance abuse counseling.
Provides assessment and referral for potential victims of abuse and/or neglect.
Intervenes when social/economic issues affect health care and disposition.
Qualifications
Required:
Current MD licensure as Registered Nurse
BSN
5 years clinical experience in patient case management / social work environment
Strong assessment skills
Ability to prioritize multiple tasks
Strong leadership skills
Ability to interact with various members of health care team
Ability to advocate for patient/family
Good verbal and written communication skills
Basic PC skills such as Windows, Outlook, etc.
