Job Description Job Summary/Purpose:
The Advanced Practice Clinician (APC) is responsible for the provision of health care services to individuals, families, and/or groups who receive their care under the auspices of Sentara Medical Group (SMG). Health care services include conducting health assessments, prescribing/providing treatments/interventions, promoting health, and providing disease prevention and management. Health care services are provided under the direction and supervision of assigned primary care physician (collaborative agreement). The APC is expected to participate in quality assurance and improvement activities, engage in member of the team activities, and use resources (human and material) appropriately.
Minimum
Qualifications:
Education: Successful completion of Master's Degree in a nationally accredited Nurse Practitioner Program or certified Physician's Assistant program. MA certification preferred.
Experience: Prior experience as nurse practitioner or physician assistant as determined by the Administratorof the specialty area; ambulatory care experience preferred.
Licensure: License as a Nurse Practitioner or Physician's Assistant in the state in which employed. Prescription writing authority/certification as provided by state law.
Key Competencies: Interpersonal skills to be an active, collaborative member of the care team; analytic ability to review internal processes that need improvement and make suggestions to the Practice Manager, Director of Operations and/or Medical Director; and the verbal ability to communicate thoughts and actions to individual patients, families, peers, and other health care team members.
Other: BLS certification; ACLS certification for Urgent Care Center. The APC maintains continuing education credits of at least 30 hours per year and maintains prescriptive authority and certification; acquires special clinical competencies as required by the group; participates in quality assurance and utilization management activities; and assists with risk management functions.
Looking for experience in Family Medicine and/or Urgent Care, possible extended hours for this is a new site.
Demonstrates the minimum knowledge, skills and abilities to care for the individualized needs of the patient to include physical, psychological, socio-cultural, spiritual and cognitive needs as well as functional abilities including the need for diversified use of such practices.
