Position Summary
Provides case management services to persons living with HIV/AIDS in a six county area. Functions as part of a multidisciplinary, comprehensive HIV service team providing case management services in clients' homes, environments or in the office.
Clinical Outcomes:
Participates in regular clinical supervision, program meetings and peer consultation.
Conducts outreach activities to expand community awareness of case management programs and services.
Patient Focus:
Completes an assessment of clients' physical, psycho-social, medical, legal and financial wants and needs, and collaborates with clients to develop services plan.
Serves as a client advocate through education, referral and support.
Schedules client contacts depending on client needs or expressed preferences, service plan, program policy and /or case load.
Assists the patient to identify and/or develop their support system and encourages the patient to utilize their support system.
Identifies and effectively utilizes community resources to meet the needs of patients/families.
Collaborates with other healthcare providers and health system administrators in order to optimize outcomes.
Works with clients to assist them in adhering to medication regimens.
Current Licenses
Licensed Social Worker (LSW) preferred
Education
Bachelor's degree required
Experience
1 year or more preferred
Other Special Skills -Preferred
Proficiency in a suite of Microsoft Office applications
Requires demonstrated knowledge of professional social work practices to ensure delivery of quality services to the community
Must be able to communicate with a wide variety of populations utilizing non-judgmental and professional communication skills
Requires sound social ethics and the ability to exercise extensive discretion and independent judgment when interacting with clients
Must have the ability to develop and maintain positive working relationships
Requires knowledge of available community resources
Must have the ability to work independently and as a member of a multidisciplinary team to deliver program servicesEstimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
