Float Pool for Montgomery County
Home Care Aides at MedStar VNA Care Partners make a positive difference by assisting in the provision of home services that reflect agency philosophy and objectives and meets or exceeds standards of home health and nursing care.
Responsibilities:

1. Clinical: Performs basic, routine and specialized tasks in assigned department, under the supervision of a registered nurse.
a. Completes job-specific functions and is available for more responsibility.
b. Provides excellent patient care as per feedback from RN Case Manager and patient/family.
c. Implements clinical care per agency policy and procedure.
d. Delivers quality care to the patient that is evidenced by good patient personal hygiene.

2. Environmental: Maintains a clean, safe, prepared work environment.
a. Maintains a patient environment that is clean, safe and healthy.
b. Takes initiative to solve problems and recommends improvements of agency processes.

3. Administrative: Assists in the reporting of accurate patient data.
a. Documents communication with professional staff
b. Reports observations of patient condition to the RN accurately and in a timely manner.
c. Participates in patient case conferences, as appropriate.
d. Communicates with the RN on a regular basis regarding assignments.

4. Development: Maintains current knowledge, certification, and licensure.
a. Identifies learning needs in self and initiates actions to address these education needs.
b. Attends continuing education programs in order to maintain certification, minimum 12 hours of year.

5. Organizational knowledge: Works effectively within and between departments.

6. Department/Unit Specific Functions and Duties
a. Consistently displays professionalism in interactions with patients, families and all staff.
b. Supports agency by providing coverage when there are shortages.
c. Consistently demonstrates appropriate decision-making skills, in a timely manner.
Qualifications:
EDUCATION:High School Diploma or equivalent and the completion of a certified nursing assistant program.
Experience: One year equivalent experience in acute care or home halth care.
LICENSE/CERT/REG: CNA Licensure required. CPR certification.
SKILLS: Effective verbal and written communication skills. Basic computer skills for data entry and ability to use computerized clinical information systems (Clinicans View and Chart View).
Experience: Minimum 1 year equivalent experience in acute care or home health care.
Reliable transportation required.
Work location: Montgomery and/or Howard countyEstimated Salary: $20 to $28 per hour based on qualifications.
