The incumbent is responsible for the management, coordination and provision of social work services to veterans and their families on the Acute Care units INCLUDING BUT NOT LIMITED TO:
Conduct psychosocial assessments to determine needs of Veterans and family members
Work with patients and families who are experiencing an emotional, medical, and/or social problems utilizing individual, group, and family counseling skills
Active participant in the treatment planning process with other disciplines
Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate
Provide consultation/education to Veterans and families on community resources, advance directives and VA benefits
Act as a consultant to other team/staff members
Provide consultation/education to internal and external stakeholders on social work practice and VA policy and procedure
Act as liaison between VA and community resources, to include marketing and public relation duties as needed
Provide case management to Veterans and families, as needed, throughout the continuum of care
Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate
Address Advance Directives and Organ Donation in accordance with the standards of their work area
Provide wellness/prevention education and facilitate patient/family support groups as appropriate
Coordinate treatment and communicate with the care lines to facilitate continuity of care for Veterans
Participate in committee assignments, as deemed necessary by the supervisor
The incumbent has working knowledge and skill in the use of word processing, data management, and other computer software Ability to learn and utilize software programs in use by the VHA
Comply with Joint Commission, VA and medical center policy and procedures, and identified performance and mission critical measures
Assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death
Document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off communication when transferring of patient care
Participate in modification of tour of duty to accommodate patient care and facility needs
Establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies
Attend all appropriate staff meetings (interdisciplinary treatment team meetings, Social Work staff meetings, and other committees as appropriate, etc) and perform other duties as required
Submits self-assessment to supervisor in a timely manner that identifies areas of competence and knowledge deficits using VA Central Office forms Self-assessments should be submitted 30 days in advance of performance appraisal due date
Utilizes the eight core competencies of the High Performance Development Model (HPDM) in practice
Is able to communicate effectively, both orally and in writing, with people from varied backgrounds
Has basic knowledge of medical and mental diagnoses, disabilities, and treatment procedures to include acute and chronic illnesses/injuries; common medications and their effects/side effects, and basic medical terminology
BASIC REQUIREMENTS:
U.S. citizenship . Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Selective Service : Males born after 12/31/59 must be registered with the Selective Service.
Background Investigation : A background investigation is required and continued employment is contingent on completion of that process to determine suitability for federal employment.
English Language Proficiency : In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position.
EDUCATION - Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited._ A doctoral degree in social work may not be substituted for the master's degree in social work._ Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
LICENSURE OR CERTIFICATION: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM websit. Exception: VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. For grades below or at the full performance level, VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within 3 years of their appointment as a social worker.
GRADE DETERMINATIONS:
In addition to the basic requirements, applicants must have One year of experience equivalent to at least the next lower grade in the normal line of progression for the occupation in the organization. Examples of experience would typically include, but are not limited to:
GS-9 - Experience, Education and Licensure - GS-9 is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-9 level are working toward completion of prerequisites for licensure or certification. In addition, the candidates must demonstrate the KSAs below. Assignments. Individuals assigned as GS-9 social workers are considered to be at the entry level and are closely supervised, as they are not yet functioning at the independent practice level conferred by independent licensure or certification. Social workers at the GS-9 entry level are typically assigned to VHA program areas that do not require specialized knowledge or experience. Since these social workers are not practicing at an independent level, they should not be assigned to program areas where independent practice is required, such as in a CBOC, unless there is a licensed social worker in the program area who can provide supervision for practice. GS-9 social workers provide psychosocial services in the assigned area under supervision.
GS-11 - Experience, Education and Licensure - Completion of 1 year of experience equivalent to the next lower grade level OR a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. Assignments. This is the full performance level for social workers. GS-11 social workers are licensed or certified to independently practice social work. They are assigned to all program areas, including inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area.
