FAMILY CARE COORDINATOR
DESCRIPTION: American House Senior Living Communities (AHSLC) and the AAA 1-B have created a care coordination model to support the ability of AHSLC to stay healthy, manage chronic conditions, and return directly home to AHSLC following an acute medical episode, including an emergency room visit or hospitalization. The Family Care Coordinator (FCC) will work with AHSLC residents and families to develop person centered recovery plans and coordinate the AHSLC partner services, including on-site service provider, skilled therapists, and private duty home care. The FCC will also work to identify community resources available through the AAA 1-B and other local providers. The FCC will be basedEstimated Salary: $20 to $28 per hour based on qualifications.
