Part-Time Nurse Care Manager - Case Management Department - Chilton Medical Center
(2-3 Weekdays Per Week / 8:30 a.m. - 4:30 p.m. & One Weekend Shift Every Other Weekend / No On-Call)
Essential Functions: The P/T Nurse Care Manager will be responsible for coordinating the effective discharge planning, reviewing patient assessments and documenting patient data. Incumbent will be responsible for utilization management, meeting daily with patients/families, participating in daily patient care rounds and communicating with physicians and the multidisciplinary team to coordinate plan of care.
Education: Graduate of a NLN Approved Nursing Program
Qualifications/Requirements: Current NJ Nursing License. Qualified candidates will possess strong oral and written communication skills, good organizational skills and be detail-oriented in nature. In addition, qualified candidates will have the ability to work both independently and collaboratively. Patient care management experience preferred.
