Medical Social Worker
Fremont Area Medical Center has an excellent full-time opportunity for aHospice Medical Social Workerto provide compassionate, excellent service to patients, residents and families. The Medical Social Worker serves as a key member of the interdisciplinary care teams performing complete and timely psychosocial assessments, care transition/ discharge planning duties, patient/family/bereavement counseling and acts as a liaison for referral to community agencies.
TheHospice Medical Social Worker requires a Masters Degree (MSW) and unrestricted license to practice as a Certified Social Worker in the State of Nebraska.Hospice experience is strongly preferred, but not required.
This is an excellent opportunity for a Medical Social Worker with strong organizational skills, excellent interpersonal skills and preference to work in a self-directed fashion to join a progressive organization and work together as a team to provide valuable medical social work services.
