ARHS CSP I - ID Support Coordinator

This position provides case management/support coordination for adults with intellectual disabilities and/or co-occurring mental health disorders eligible for Targeted Case Management. Works as part of a case management/support coordination team to ensure Medicaid, Licensure and Department of Justice standards are met. Coordinates and writes Person Centered Plans, Plans for Support, makes referrals as necessary and coordinates supports with other providers. Some evening work may be required
Bachelor's degree in Human Services related field is required. Must meet the knowledge, skills and abilities for Case Manager as set forth by the Virginia Medicaid. Prefer applicants with at least one year of experience working with individuals with mental intellectual disabilities but will consider individuals who have at least 9 months of applicable experience. Certification in CPR and First Aid are required (training provided). Must have an acceptable driving record.
