Summary:
The Certified Wound Ostomy and Continence Nurse (CWOCN) is a professional, registered nurse who demonstrates expertise in a specific area of clinical nursing that has been further developed through a certified WOCN program. Using principles of evidenced based nursing practice, the CWOCN focuses on improvement of processes, treatments, plans of care and other related interventions and makes recommendations to improve the delivery of high quality care to patients with wounds, ostomies, pressure ulcers, fistulas, chronic and/or draining wounds and incontinence patient populations. The CWOCN role models critical thinking by demonstrating active and proactive problem-solving; teaches the techniques and application of critical thinking skills. The CWOCN will serve as a resource for nurses and other healthcare professionals / providers for neonate to geriatric age groups.

Essential Functions &
Responsibilities:
Uses the nursing process to provide for the physical, mental and social well being for individuals and families by assessing, planning, implementing and evaluating results.
Makes home visits to assess patient's needs and develop nursing diagnosis per Home Health Assessment and Admission Process and Referral Process.
Implements established wound/ostomy/continence treatment plan of care.
Contacts referring physician to develop the Plan of Care (POC).
Notifies physician of identified changes in patient's condition.
Works with staff to address questions and/or demonstrate techniques related to defined treatment plans.
Documents findings/assessments and orders in the medical record.
Evaluates ongoing effectiveness of treatment plans.
Refers patient to appropriate community services and/or resources.
Promotes continuity of care between outpatient and inpatient setting through consultation with patients, families and health care providers.
Works closely with patient/ family, healthcare team to identify discharge needs or services and obtains physician concurrence with discharge plans.
Documents findings/action in the medical record to ensure continuity of care.
Assists in Annual Skills Validation and orientation process for staff in collaboration with Education Coordinator and Clinical Managers.
Works closely with patient/family to coordinate and enhance the care of patient populations at high risk for complications related to integumentary system.
Reviews and recommends care based on the established treatment plans for defined patient populations.
Develops, implements and updates a plan of care that addresses the patient's needs as per the comprehensive assessment.
Makes follow-up home visits to provide continuing educational and treatment needs.
Plans, implements and evaluates teaching programs for patients/families.
Works with patient/family and healthcare team to ensure that knowledge and skills related to established treatment plan is sufficient for assumption of self/family care prior to discharge.
Documents findings in the clinical record related to teaching effectiveness and patient / family understanding/skill demonstration.
Provides written teaching tools/literature as appropriate (i.e. cultural and age-specific) and documents in clinical record.
Documents in the clinical record barriers to completion of teaching.
Provides input into the development of teaching protocols.
Demonstrates a commitment to the profession of nursing and self-development.
Contributes to the development and/or refinement of wound, ostomy or continence treatment protocols or care management strategies.
Participates in identified committees to identify opportunities for improving patient outcomes.
Participates in performance improvement activities.
Collaborates with the Performance Improvement Manager to develop and implement improvements
Completes and corrects OASIS information prior to submission to state.
Submits monthly Quality Control Records.
Leads the development and revision of clinical standards and protocols applying evidence-based principles and recent research.
Serves as an educator as well as a mentor to Home Health staff.
Provides coaching and mentoring regarding decisions about quality, cost, effective care and customer service/satisfaction.
Orients new Associates and nursing students to the role of the WOCN.
Evaluates existing programs and proposals and recommends methods and approaches to enhance efficiencies and improve patient care processes and outcomes.
Obtains evidenced based/cost effective clinical equipment/products which support the care of the Home Health patient population by partnering with Materials Management.
Remains current in medical surgical care by reviewing journal articles and other professional literature, attends professional conferences, training and site visits to remain up to date on trends and new and emerging issues. Shares this information and assists with implementing application of learned information throughout departments as appropriate.
Participate in multidisciplinary, staff and administrative meetings.
Other duties as assigned.
Other Functions:
The CWCON functions as a clinical resource, providing clinical, professional, educational in-services, revisions of policies and procedures and quality improvement with standardization of ostomy and wound products to MWHC Associates in pursuit of improved delivery of care and patient outcomes. The scope of practice encompasses the use of the nursing process in practice, education, consultation, research and administration.
Qualifications:
BSN with a clinical focus is required.
Registered Nurse licensed to practice in Virginia.
Graduation from a WOCN accredited Enterostomy Therapy Nurse Educational program.
One year of recent medical surgical nursing practice with certification on Wounds Ostomies and Continence or eligible to become certified within twelve months of hire.
Must be Board Certified in Nursing through WOCN.
Must maintain competency/certification in BLS and other competencies as required.
Member and active participant in a professional nursing organization.Estimated Salary: $20 to $28 per hour based on qualifications.
