Knowledgeable in stages of human growth and development for neonate, pediatric, adult and
geriatric populations.

and written communication.
Ability to interact with other professionals as part of a multidisciplinary team.

SCOPE OF AUTHORITY / FUNCTION:
Facilitates safe and timely discharge of patients requiring post acute care services, or special transportation in coordination with care management team.
Coordinates the legal process and paperwork involved with: involuntary patient admissions, protective services, guardianships and adoptions.Estimated Salary: $20 to $28 per hour based on qualifications.
