BASIC PURPOSE
In an environment of continuous quality improvement, assesses community clients'' learning and health needs, provides outpatient dietary counseling, provides health/wellness education and support for clients, provides DSME for Diabetes Education Center and initiates referrals to appropriate resources.
PRINCIPAL ACCOUNTABILITIES
1. Impact wellness programming with focus on prevention.
2. Conduct outpatient diabetes education/counseling.
3. Provide nutritional counseling.
4. Teach health classes.
5. Perform other related duties.Estimated Salary: $20 to $28 per hour based on qualifications.
