The Per Diem Hospice Social Worker works in conjunction with the Interdisciplinary Hospice Team in providing supportive services to patient and family. The Social Worker is responsible for direct service to terminally ill patients and their families and is responsible for visiting assigned hospice patients and participating in initial visit whenever possible.
Provides a comprehensive assessment of assigned patients in a timely manner and develops a social work treatment plan based upon the assessment.
Responsible for accurate and timely documentation and assists team members in the selection of, referral to, and use of appropriate community resources.Estimated Salary: $20 to $28 per hour based on qualifications.
