Location Description
St. John Macomb-Oakland Hospital, Macomb Center, is a 376-bed facility offering a wide range of inpatient and outpatient services. Our Centers of Excellence include Behavioral Medicine Services, Cancer Care, Cardiology Services, Emergency Center, Physical Medicine and Rehabilitation Services, Surgical Services and Women''s Health Services and our state-of-the-art Webber Cancer Center.
Job Summary
Under limited supervision, is responsible for coordinating the health care plan for assigned patient population through the use of care plans, critical pathways, managed care guidelines, utilization review processes and collaboration with all members of the health care team. Evaluates care, utilizing clinical skills, based upon quality, access, and cost-effectiveness. Maintains the continuum of care through the coordination and integration of all phases of patient care. Demonstrates unit/area clinical designated competencies. Performs utilization review of hospital stays in accordance with the hospital''s Utilization Management Plan, Quality Management Plan and third party payor requirements. Provides clinical expertise, consultation services and education to all members of the health care team in order to ensure quality and cost effective care.
Required
Current license to practice as a Registered Nurse in Michigan
Graduate of an accredited School of Nursing
Three years clinical experience
Advanced interpersonal skills are needed in order to discuss factual information with physicians, hospital staff, patients and families and insurance agencies
Able to participate on hospital and system wide committees''.
Able to work within time constraints and job demands.
Preferred
Bachelor''s degree
One year experience in case management, utilization review or discharge planning.
