The Inpatient Care Manager, in collaboration with the clinical team and medical provider, provides discharge planning coordination and intervention. The Care Manager strives to promote patient wellness, improved care outcomes, efficient utilization of health services and minimize denitals of payment among a patient population with complex health needs.
Requirements:Basic professional knowledge; equivalent to a Bachelor''s degree; working knowledge of theory and practice within a specialized field.BSN required. This requirement will be lifted for LBH candidates employed as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager. 3-5 years experience.Knowledge of insurance and governmnet requlations;Negotiation and relationship building skills; Excellent problem-solving ability; Microsoft Office Suite; Effective Leadership skills; Acts in accordance with LifeBridge core to achieve MD licensure if out of state, Maryland Registered Nurse License; American Heart Association CPR Certification; Case Mgmt cert required 3-5 yrs of hire for role.Demonstrates the ability to follow verbal instructions, as well as the ability to communicate effectively both verbally and in writing.Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
