This Social Worker is assigned to the HBPC Program with responsibility to provide direct services for adult and geriatric veterans enrolled in this program. He/she is responsible for assessment, planning, and provision of case management services in the HBPC Program. This professional social worker coordinates a variety of activities including outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to Medical Center staff and community based service providers, the collection and maintenance of statistical data, program planning evaluation and modification, and the supervision fo students as assigned. As a member of the Interdisciplinary Treatment Team in the HBPC Program, the individual conducts work activity independently using his/her educational training, clinical experience, VA policies, and Joint Commission standards in administering direct services: psychosocial assessment of patient/family needs in relation to the provision of interdisciplinary treatment planning and service delivery; individual, group, and family counseling and casework services for the purpose of addressing identified needs. He/she will provide services both via telephone contact and in the homes of veterans enrolled in HBPC. The incumbent will collaborate with the other members of the HBPC team in the provision of comprehensive health care services to HBPC veterans, will ensure equity of access, service, and benefits to this population. The social worker will conduct training for staff, students and community providers regarding HBPC issues, will provide education, information and liaison with resources for veterans in the community, and will provide technical assistance and educational sessions to community organizations as needed. He/she is responsible for the implementation of written treatment program policies and procedures developed by Social Work Service that are based on coherent empirical and theoretical rationale and are consistent with patient rights and the maintenance of comprehensive written patient progress notes, outcomes and discharge planning services in collaboration with the unit treatment team. Other duties and responsibilities may be assigned as needed for the benefits of the veteran, family and quality of services provided.
Work Schedule: Monday - Friday 7:45am - 4:30pm nights/weekends as needed

Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Education. Have a master''s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master''s degree in social work. Licensure. Persons hired or reassigned to social work positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master''s degree level. SPECIALIZED EXPERIENCE FOR GS-11 LEVEL: Requires a minimum of one year of post-MSW degree professional social work experience in the field of health care. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently. (A clinical setting is a medical center, clinic - medical or psychiatric, residential treatment center, or any other type of facility where social work participates in a collaborative treatment and is identified with the medical profession.) Experience must have been obtained AFTER earning a Master''s Degree in Social Work.
