_NATIONAL GUARD MEMBERSHIP IS REQUIRED:_ This is an excepted service position that requires active military membership in a compatible military position in the Hawaii Army National Guard (HIARNG). Selectee will be required to wear the military uniform. Acceptance of this position constitutes concurrence with these requirements as a condition of employment. _LOCATION:_ Office of the State Surgeon (OTSS), Medical Detachment Hawaii Army National Guard, Kapolei, Hawaii. _AREA OF RECRUITMENT:_ Present commissioned officer in the grade of O3 and below of the US Armed Forces.

_SUMMARY OF DUTIES:_
The primary purpose of this position is to assist the OTSS in accomplishing the overall health and medical readiness program goals of the Hawaii Army National Guard.
Serves as a Nurse Case Manager.
1. Functions as a case manager and/or team leader for National Guard members, as well as those awaiting Medical Evaluation Board and those returning to duty that require management of health care services.
2. Based upon triaging, comprehensive intake, and ongoing monitoring, the case manager facilitates entry into the appropriate level of care, at the appropriate level of clinical urgency and within TRICARE access standards.
3. Facilitates communication and coordinates medical services with the WTU primary care manager (PCM) and other members of the health care team and medical facilities.
4. Works in close and effective collaboration with OTSS, physicians and other healthcare and military personnel independently performing assessment, review, and analysis of cases with an established timeframe after identification for intake.
5. Participates in case management and joint planning with OTSS, Health Care Providers, Health Benefits Advisors, Health Care Finders, and Utilization Review officials in military and civilian health care facilities identify beneficiaries and beneficiary families with long-term health care needs and/or high risk, and high cost diagnoses.
6. Participates as an active member of the Warrior interdisciplinary treatment team.
7. Develops, in collaboration with nursing leadership, outcome instruments to be used to measure clinical productivity.
Performs Nurse Educator Functions.
1. Coordinates, implements, and evaluates the efficacy of the individualized treatment plans for selected high cost/high risk soldiers.
2. Provides counseling and continued consultative support to soldiers and their families regarding disease process, rehabilitative needs and health care management.
3. Oversees daily interactions with assigned soldiers; telephone triage; referrals made to other services within the hospital and liaison between referral sources and the program, documentation and chart maintenance.
Performs other duties as assigned.

_MINIMUM Requirements_ : Candidates at all grades must have active, current registration as a professional nurse and meet one of the following requirements:
(1) Graduation with a bachelor's or higher degree in nursing OR
(2) Graduation from a 3-year (at least 30 months) diploma program of professional nursing OR
(3) Graduation from an associate degree program or other program of at least 2 years in a school of professional nursing
AND
A degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Applicants must have active, current registration as a professional nurse in a State, District of Columbia, the Common-wealth of Puerto Rico, or a territory of the United States.
_SPECIALIZED
Experience:_ Must have at least 36 months of specialized experience education, or training in the full range of professional nursing care principles, practices and procedures. Experience in assessing, planning, implementing and evaluating patient care utilizing the nursing process. Experience acquired from training or education using health care equipment and techniques common to normal nursing practices. Develops and implements health protection plans, informational material, SOPs, policies, procedures and regulations. Experience in compiling statistical information, maintaining records and reports.
_MILITARY TECHNICIAN COMPATIBILITY POSITION:_ Prior to placement in this position, the selected applicant must be assigned to a valid position in the unit of employment, and be assigned militarily in 66H, Nurse.

_SUBSTITUTION OF EDUCATION FOR SPECIALIZED EXPERIENCE_
Completion of all requirements for a doctoral degree (PH. D. or equivalent) or three full academic years of graduate education. Combination of professional experience and graduate education: Equivalent combinations of professional experience and graduate education of the types described under Experience are acceptable.

g. Final selection will be based upon qualifications, suitability, and available manpower resources.
h. This employer will provide the Social Security Administration (SSA) and if necessary, the Department of Homeland Security (DHS) with information from each new employee's Form I-9 to confirm work authorization.
i. Selectee may be scheduled for formal training at the Professional Education Center (PEC) within the first six months of employment. Unless there are extenuating circumstances, failure to complete National Guard Bureau prescribed courses at PEC within the first year of employment, may be cause for reassignment to other full-time positions or termination of employment.
j. Recruitment or Relocation bonus may be offered. Authorization for payment will be granted only after determination is made that payment is in the best interest of the Hawaii National Guard and approved by National Guard Bureau.
