CHL''s Supported Housing Program provides outreach support services to sixty (60) residents who all live in their own apartments throughout the city of Worcester. Individual treatment plans are developed for each resident and staff visit residents at their apartments and provide home-based and community-based support and advocacy services. Staff is available to residents 24 hours per day via pager. All residents are DMH referred.
1. Treatment Planning: Develop, implement and document, treatment plans, service notes, and quarterly reviews.
2. Community Living: Work with consumers to assist with activities of daily living and foster a sense of safety in the community which includes personal hygiene and self-care skills, money management and budgeting, symptom management, relapse prevention, use of public transportation and transporting, problem solving and conflict resolution and other interventions as necessary.
3. Medications: Monitor medication, refills, medications changes, and maintain and comply with regulations for DPH MAP certification.
4. Routine Paperwork: Prepare daily log entries, incident reports, and other required paperwork to ensure the program meets all required standards.
5. Continuity of Care: Coordinate and attend treatment teams, case conferences, and collaborate with other providers for designated consumers.
6. Coordinate Referrals: Screen and evaluate referrals, ensure all required documentation is received, and engage new consumers into the program.
7. Treatment Contracts: Assist the Program Coordinator in the development and implementation of treatment contracts, i.e. Recovery Contracts, Behavioral Plans, etc.
8. Case Coordination: Act as a liaison with other service providers by keeping them informed of client's mental and physical health.
9. Crisis Management: Coordinate appropriate level of care for consumers who are in crisis.
10. Other duties: Other related duties as assigned in order to provide quality rehabilitation services, will include transporting residents in personal car.
11. Maintain Competencies: Human Rights, Dual Diagnosis Infection Control, MAP, CPR, First Aid, Agency and Divisional Policy and Procedure as well as other required competencies
Education and/or
Experience:
Master's Degree in social work or related human service field; or a Bachelor's degree in a related field plus 2 years experience
Competency:
1. Effective verbal and written communication with consumers, peers, supervisors and other professionals.
2. Ability to observe, document, teach and respond to mental, and physical health, as well as other needs of consumers, such as community living skills, medication education, and crisis management.
3. Display strong planning, organizing, and problem solving skills, in order to coordinate crisis management needs.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
