You will have an exciting opportunity to be part of the implementation and operation of Accountable Care Organizations (ACOs) aimed at increasing care coordination, improving quality of care and reducing costs for Medicare members. ACOs are designed to achieve the three part aim of better care, improved health and lower per capita costs. Improved health will be accomplished through prevention and chronic care management. The aim of the ACO is intended to reduce the trend of cost increases associated with the Medicare fee for service population.

CHS is seeking a Regional LCSW Care Coordinator to join this new and exciting program. The LCSW Care Coordinator provides either site visits or telephonic disease management, case management and/or utilization management services for covered members. This individual focuses on member engagement, education and empowerment to self-manage their chronic health conditions. The LCSW supports and encourages healthy lifestyle choices, health promotion, and behavioral change through motivational interviewing and incorporating behavioral change theories to reduce long term effects of chronic disease. This position will be based remotely from a home office and requires travel throughout Maryland and Northern Virginia.

Essential Functions:
o Provides telephonic and/or on-site outreach, disease management and/or case management, education and other clinically based activities to plan members.
o Conducts assessments, develops treatment plans and interventions and sets goals for treatment plans/behavioral modification within the scope of license.
o Collaborates with patients medical/health and community based providers (including the PCP, HCH, certified dieticians, behavioral health specialists, or others as needed) regarding the member's treatment needs and plan of care.
o Collects clinical data and enters information into database for tracking and metrics purposes.
o Monitors and evaluates the services and community based resources necessary to respond to the individual member's health needs.
o Completes other special assignments and projects as assigned.

Education:
o Master's prepared in Social Work required.
o Must have current, unrestricted license in Maryland, such as LCSW or LCSW-C.

Qualifications:
o Minimum 3 years clinical experience required.
o Previous experience in a care management, quality improvement, disease management and/or case management preferred.
o Knowledge of Medicare/Medicaid.
o An understanding of adult learning and behavior change principles preferred.
o Knowledge of community based resources.
o Strong clinical assessment skills, consistent with state licensure scope of practice requirements.
o Excellent written & verbal communication skills.
o Strong telephonic interviewing skills.
o Strong prioritization and organizational skills.
o Ability to assess patient health status, consistent with state licensure scope of practice requirements.
o Ability to integrate data and evaluate to design meaningful interventions.
o Must be able to prioritize and target interventions, consistent with state licensure scope of practice requirements designed to support long term behavioral change.Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
