Responsibilities: Support planners are responsible for providing support to participants with physical disabilities and are the primary point of contact with participants. Support planners perform on-site visits, information gathering, monitoring of plans of service, and completing telephonic care coordination with participants Support planners:
Collects information to assist the development of plans of service provided to our participants.
Accesses and records client and community resource information and care coordination activity.
Secure information such as psychological, medical, and social factors contributing to our participants situation, and evaluates these and participants capacities to assist in the development of plans of service for coordination services.
Provide consistent telephone contact with our participants, their family members and caregivers to assist and provide support with the coordination of services while monitoring the existing plans of service for the participant.
Manage multiple priorities and tasks with flexibility to enhance support planning servicesEstimated Salary: $20 to $28 per hour based on qualifications.
