Provides personal assistance and support per the approved PASS plan to enhance the ability of children with special health care needs to live and participate as valued family and community members, thereby fostering their independence. Plan implementation of goals includes the areas of accomplishing activities of daily life, making self-preserving decisions and participating in social roles and social settings.
Implements direct personal assistance goals developed by the family in conjunction with agency with the individual.
Provide safe one to one care per the plan for children with special health care needs as directed by parents/caretakers.
Provide socialization opportunities within home and/or outside of home when appropriate.
Perform all necessary self-care tasks as needed by the individual supported.
Carry out specific tasks outlined in the approved PASS plan.
Documents progress on required data sheets and progress notes which are made available to family and clinical consultant
Participates in family provided training's on modifications to service plan and utilizes technical assistance/expertise offered by clinical consultant to family for service improvements
To be determined by each family per State regulations. Must be at least 18 years of age. Appropriate background investigation results and clearance from Department of Children Youth and Families. Drivers must have current vehicle registration and valid State inspection, no prior accidents within one year and no history of driving under the influence. All duties will vary depending on the specific needs of each child. Parent/caretaker will provide all training specific to their child. Safety plan must be reviewed and implemented when necessary.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
