Summary:The Diabetes Management Educator/eConsult Nurse Coordinator provides individual and group diabetes self management education and serves as a resource for the nursing staff and providers relative to diabetes education and basic nursing care of the patient. This nurse coordinates and facilitates patient care with eConsult specialty providers and staff and demonstrates the knowledge and skills necessary to provide care for the population served. Experience: Bachelor''s degree from a four-year college or university preferred. Experience in diabetes patient care and/or diabetes education preferred. CERTIFICATION, LICENSURE, and/or REGISTRATIONS: Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
